Steps To Assessment Process:

Fill out all the questions using the scale provided
- Never Happens =1
- Every Month/Minor/Mild/Rarely =2
- Every Week/Moderate/Occasionally=3
- Daily/Severe/Frequently=4
Make sure you fill out all 4 pages until you get to the button that says “Finish Quiz”
Review your categories and notice which category has the worst score ( the higher the percentage shown, the more that area needs improvement)
Use the provided PDF in our resources to help you make changes in that one area.
