Steps To Assessment Process:

  1. Fill out all the questions using the scale provided
    • Never Happens =1
    • Every Month/Minor/Mild/Rarely =2
    • Every Week/Moderate/Occasionally=3
    • Daily/Severe/Frequently=4
  2. Make sure you fill out all 4 pages until you get to the button that says “Finish Quiz”
  3. Review your categories and notice which category has the worst score ( the higher the percentage shown, the more that area needs improvement)
  4. Use the provided PDF in our resources to help you make changes in that one area.
