This is your opportunity to find out if there are areas of your health needing attention to support your stress management journey.

Steps To Assessment Process:

  1. Fill out all the questions using the scale provided
    • Never Happens =1
    • Every Month/Minor/Mild/Rarely =2
    • Every Week/Moderate/Occasionally=3
    • Daily/Severe/Frequently=4
  2. Make sure you fill out all 4 pages until you get to the button that says “Finish Quiz”
  3. Review your categories and notice which category has the worst score
  4. Use the provided PDF to help you make changes in that one area
